Referrals to Tresillian

Tresillian is conducting a survey of health professionals that refer to our services. Tresillian has made changes to how we admit and manage our clients since the last time we undertook an external health professional’s survey.
Tresillian values your feedback on our services and the information gained from this survey will assist us in reviewing current services and for future service planning and development.
The survey should take no more than 10 minutes, and is completely anonymous. Please help us improve our service and take the survey.
If you are a health professional making a referral to Tresillian:
- Please ensure you are using the Health Professional Referral Form (TMR37). If you do not have access to a printer, please contact Tresillian’s Centralised Intake on (02) 4734 4400 and a copy can be faxed to you
- All sections of the Health Professional Referral Form (TMR37) need to be fully completed. Please write clearly on referrals particularly phone numbers as faxing reduces the clarity of the referral information
- Once the Health Professional Referral Form (TMR37) is completed, fax it to Fax: (02) 4734 4401
A few points to consider regarding submitting a Referral Form
- Please tick the box at the bottom of page one, which Centre the family is to attend. Ask the family to phone the Centre number and book their Pre Admission Interview (PAI) after 3 working days. This interview is conducted over the phone
- Tresillian will not contact families to arrange a Pre Admission Interview unless the referral is URGENT
- Families being referred to Residential units need to have attended a Secondary Service or currently be under the care of a Paediatrician. Example of Secondary Service would be:
- Family Care Cottages
- Day Stay Unit
- Outreach Service
- Paediatrician
- Please indicate on the form which Secondary Service they have attended
- Until the family has attended a Secondary Service they will be unable to attend the Residential Service
- If there is no Secondary Service or equivalent available in their area then the family can be referred directly to the Residential Unit by a Child and Family Health Nurse or General Practitioner
- Please note each child requires a separate referral form, e.g. twins, siblings etc
Day Stay and Outreach referrals:
- A nurse from the Centre the family will be attending will call the parents to arrange an appointment
- Day Stay is area bound for Tresillian Canterbury, Nepean and Wollstonecraft therefore the family must live in the area to attend
- Outreach is also area bound and takes in the lower north shore and inner west

Department of Community Services (DoCS) referrals:
- DoCS case workers need to contact Centralised Intake - (02) 4734 4400 and request DoCS referral forms.
- The on-line Referral Form should only be used for Brighter Futures clients
- These can be emailed or faxed
- Once a DoCS referral is received, processed and sent to the relevant Centre, the Nurse Unit Manager will contact the Case Worker to arrange a pre-admission interview
Telephone Referrals:
- Services that do not have access to a fax/email can still phone through the referral Monday to Friday 9am – 4.30pm.
Once the referral has been processed all enquiries need to go to the Tresillian Centre the family are attending.
